We understand that you may have been waiting a long time to see a specialist regarding your medical needs. A productive initial consultation, however, requires considerable preparatory work. In order to serve you best we obtain records for all the previous treatment you have received that is relevant to your condition and our practitioners review these records to provide you with a personalized and innovative care plan.
Please select the type of new patient visit you are seeking (see below) and do your best to provide complete and accurate information. Once completed either email the forms back to us at FCONP@womenscare.com or print them out and mail to us at:
The Fertility Center of Oregon
590 Country Club Parkway, Suite A
Eugene, Oregon 97405
Attn: New Patient Registration
If you have any questions about the process please call us at 541-683-1559.
Once we receive the completed forms we will contact you to schedule a consultation visit. Although your needs may feel urgent, appointments are given on a first come, first serve basis. It is not uncommon for there to be a wait to see a subspecialist like Dr. Austin. We will do our best to see you as soon as we can. We appreciate your patience.
Your initial consultation is generally 60 to 90 minutes long. We urge both you and your partner to attend. Your history will be explored in detail. A diagnosis and initial treatment plan is sometimes possible by the end of the consultation. Sometimes the diagnosis is not clear and next steps in making a diagnosis will be outlined. Lab work, imaging studies and surgery are all possible diagnostic tools. We will provide written instructions and informational handouts to make your next steps more clear.
We understand additional questions may occur in the days and weeks following your initial consultation. Our coordinators and support staff are knowledgeable about the types of care we provide and your treatment plan is accessible to them.
Please call with any urgent questions or use our electronic patient portal for secure messaging for all other questions.
