
We understand that you may have been waiting a long time to see a specialist regarding your medical needs. A productive initial consultation, however, requires considerable preparatory work. In order to serve you best, we obtain records for all the previous treatment you have received that is relevant to your condition and our practitioners review these records to provide you with a personalized and innovative care plan.
To efficiently prepare for your visit, please contact us at 541-683-1559, Option 3, to have our team send you a New Patient Packet via DocuSign. These packets must be completed prior to scheduling.
Once we receive the completed forms, we will contact you to schedule your appointment. Your needs are urgent and your appointment will be scheduled on a first-come first-serve basis after receiving the information requested. We will do our best to see you as soon as we can and we appreciate your patience.
Your initial consultation is generally 60 to 90 minutes long. We urge both you and your partner to attend. Your history will be explored in detail. A diagnosis and initial treatment plan are sometimes possible by the end of the consultation. Sometimes the diagnosis is not clear and the next steps in making a diagnosis will be outlined. Lab work, imaging studies, and surgery are all possible diagnostic tools. We will provide written instructions and informational handouts to make your next steps more clear.
We understand additional questions may occur in the days and weeks following your initial consultation. Our coordinators and support staff are knowledgeable about the types of care we provide and your treatment plan is accessible to them.
